Required Health Forms

Required School Documents

✅ Returning Students - If your child is entering a grade that requires updated forms (Kindergarten, 2nd, or 6th), be sure to submit the appropriate documentation before the first day of school.

✅ New Students - New families must submit a Release of Records Form within 7 days of receiving enrollment offer. Also, please submit proof of all previous health exams before the first day of school.

🩺 Immunization Form - Required for students entering Kindergarten and 6th Grade.

🦷 Dental Exam Forms - Required for students entering Kindergarten, 2nd Grade, and 6th Grade.

👁️ Vision Exam Form -Required for students entering Kindergarten.

In accordance with Illinois law, all children beginning school or who are entering kindergarten and sixth grades need to submit a current Immunization/Medical Examination form.

Dental Form

In accordance with Illinois law, all children beginning school or who are entering into kindergarten, the second, and sixth grades need to submit a current Dental Examination Form.

Vision Form

Illinois law requires that proof of an eye examination by an optometrist or physician (such as an ophthalmologist) who provides eye examinations be submitted to the school for students entering Kindergarten.

The AOA (American Optometry Association) recommends an eye exam every two years if no vision correction is required. Children who need eyeglasses or contact lenses should be examined every year.
